Public Health and Skin Color

Skin color is a visible human characteristic produced largely by differences in the amount, type, and distribution of melanin in the skin. Human pigmentation evolved in response to environmental pressures, particularly ultraviolet radiation, but its importance to public health extends far beyond biology. Skin pigmentation can influence responses to ultraviolet radiation, vitamin D synthesis, the appearance of disease, the performance of some optical medical devices, and the recognition of clinical symptoms. At the same time, socially assigned meanings attached to skin color can influence discrimination, healthcare access, socioeconomic conditions, mental health, and exposure to harmful practices such as skin bleaching.

Modern public-health research increasingly distinguishes skin pigmentation itself from socially constructed racial and ethnic categories. People within the same racial or ethnic group can have widely different skin tones, while people assigned to different racial categories can have similar pigmentation. This distinction is particularly important when studying medical technologies, disease presentation, ultraviolet exposure, and other processes in which the physical properties of skin may directly affect measurement or clinical recognition.

Skin Pigmentation as Biology and Social Experience

Human skin pigmentation represents an interaction among genetics, geography, ultraviolet radiation, migration, diet, and cultural behavior. Darker pigmentation generally provides greater protection against ultraviolet radiation, while lighter pigmentation can permit more efficient cutaneous vitamin D production under conditions of relatively weak ultraviolet exposure.

These adaptations developed over long periods of human evolution. Modern migration, clothing, indoor lifestyles, dietary changes, and differences in sun exposure have altered the environments in which many pigmentation patterns originally evolved. Consequently, populations may experience environmental conditions very different from those that shaped their ancestors' pigmentation.

Skin color also has powerful social consequences. In many societies, skin tone influences how people are perceived and treated. Colorism—preferential or discriminatory treatment based on relative skin tone—can operate both within and between racial and ethnic groups. Research has associated color-based discrimination with differences in physical health, psychological distress, sleep, self-rated health, socioeconomic opportunity, and other aspects of well-being.

For public health, skin color therefore occupies two distinct but interacting domains: pigmentation is a biological characteristic with physiological consequences, while perceived skin color can also function as a social determinant of health.

Medical Diagnosis and Recognition of Disease

Many medical signs are traditionally described using color changes that may be easier to recognize on lightly pigmented skin. Redness, pallor, cyanosis, inflammation, bruising, jaundice, and some forms of tissue injury may appear differently or be less visually obvious in darker skin.

These differences can affect clinical assessment. Conditions such as eczema, psoriasis, lupus, rosacea, pressure injuries, and inflammatory disorders may present with different visible characteristics across the pigmentation spectrum. In darker skin, inflammation may appear purple, gray, brown, or otherwise less conventionally "red." Post-inflammatory hyperpigmentation may also be a prominent or persistent consequence of skin disease.

Pressure injuries illustrate the problem particularly well. Traditional descriptions often emphasize non-blanching redness, but this sign may be difficult to identify in deeply pigmented skin. Effective assessment may therefore require greater attention to temperature, texture, swelling, tenderness, firmness, and other non-color indicators.

Jaundice in newborns presents another challenge because visual recognition of yellow discoloration can be unreliable in darker skin. Researchers are investigating how pigmentation affects both visual examination and noninvasive bilirubin measurement.

Pulse Oximetry and Optical Medical Devices

Pulse oximeters estimate blood oxygen saturation by transmitting light through or reflecting light from tissue. Because melanin also absorbs light, researchers have investigated whether pigmentation affects the accuracy of these devices.

Multiple studies have reported that some pulse oximeters can overestimate arterial oxygen saturation in people with darker pigmentation, particularly when actual oxygen levels are low. This can contribute to "occult hypoxemia," in which a patient has clinically significant low blood oxygen despite apparently reassuring pulse-oximeter readings.

The issue became especially important during the COVID-19 pandemic, when pulse oximetry was widely used to guide treatment and hospitalization decisions. Subsequent research has emphasized the need for improved device testing, better calibration across the full range of human pigmentation, and objective measurement of skin tone rather than treating racial identity as a substitute for pigmentation.

Similar concerns apply to wearable devices that use photoplethysmography to estimate heart rate, oxygen saturation, and other physiological measurements. The growing use of smartwatches and medical wearables makes equitable performance across skin tones an increasingly important engineering and public-health objective.

Artificial Intelligence and Medical Imaging

Artificial intelligence is becoming increasingly important in dermatology, medical imaging, and diagnostic decision support. However, AI systems depend heavily on the data used to train them.

Dermatology image collections have historically contained disproportionate numbers of images from lighter-skinned patients. As a result, both clinicians and computer models may have less experience identifying diseases in darker skin.

Studies of dermatology AI have found performance differences across skin tones, and researchers have demonstrated that more diverse training data can reduce some disparities. Generative artificial-intelligence systems can reproduce similar biases when asked to create medical images, frequently producing lighter-skinned examples unless diversity is specifically incorporated into the system or prompting process.

Medical-imaging technologies such as thermal imaging and hyperspectral imaging may also encode information related to pigmentation. Developers therefore need to evaluate whether skin tone becomes an unintended source of bias in diagnostic algorithms.

Medical Education and Representation

A recurring theme in the research is inadequate representation of darker skin in medical education.

Dermatology textbooks, surgical textbooks, teaching collections, and clinical training materials have historically shown relatively few examples of diseases in deeply pigmented skin. This matters because clinicians often learn visual diagnosis by repeated exposure to representative images.

Studies of medical students and dermatology residents have found gaps in education involving skin of color. Training programs specifically designed to increase exposure to diverse skin tones generally improve diagnostic knowledge, confidence, and recognition.

Improving education requires more than adding a small number of photographs. Medical curricula need to explain how inflammation, infection, malignancy, vascular changes, autoimmune disorders, and other diseases may appear across the full pigmentation spectrum.

Dermatologic Disease and Health Disparities

Many dermatologic conditions affect people across all skin colors, but prevalence, severity, clinical presentation, treatment access, and quality-of-life consequences may differ among populations.

Atopic dermatitis can produce substantial disease burden in populations with darker skin, while conventional measures of erythema may underestimate its severity. Psoriasis may also appear differently, complicating diagnosis and severity assessment.

Acne can produce persistent post-inflammatory hyperpigmentation and scarring in darker skin, making control of inflammation especially important. Hidradenitis suppurativa has been reported to impose a disproportionately high burden on some Black populations and can be compounded by delayed diagnosis and limited specialist access.

Vitiligo can be particularly visible on highly pigmented skin because of the contrast between depigmented and surrounding areas. Research documents significant emotional and quality-of-life effects among patients with darker skin.

Rosacea has historically been considered uncommon in darker skin, but evidence suggests that it may often be overlooked because characteristic redness and visible blood vessels are harder to recognize.

Skin Cancer and Melanoma

Higher concentrations of melanin provide significant protection against ultraviolet radiation and are associated with lower overall rates of many ultraviolet-related skin cancers. However, lower incidence does not mean absence of risk.

Melanoma and other skin cancers occur in people with all skin tones. Research has documented disparities in melanoma diagnosis, stage at presentation, treatment, and survival among racial and ethnic populations.

Acral lentiginous melanoma is particularly important in discussions of skin cancer and skin color. This melanoma subtype occurs on areas such as the palms, soles, and beneath the nails and constitutes a larger proportion of melanoma cases among Black, Asian, and Hispanic populations than among White populations.

Later diagnosis, lower public awareness, access to dermatologists, socioeconomic factors, clinical education, and differences in tumor biology may all contribute to disparities in outcomes. Public-health messaging therefore needs to balance two facts: darker pigmentation substantially reduces the risk of many ultraviolet-related skin cancers, but people with darker skin can still develop serious and sometimes fatal malignancies.

Ultraviolet Radiation, Vitamin D, and Photoprotection

Melanin absorbs ultraviolet radiation and reduces ultraviolet penetration into the skin. This protection lowers susceptibility to sunburn and many forms of ultraviolet damage, but it also reduces the efficiency of ultraviolet-B-driven vitamin D synthesis.

Studies have repeatedly identified relationships among pigmentation, latitude, season, sun exposure, diet, supplementation, migration, and vitamin D status. Darker-skinned populations living at high latitudes may face greater risk of low vitamin D concentrations, particularly during seasons with limited ultraviolet-B radiation.

Public-health recommendations must therefore avoid simplistic assumptions. Skin color is only one factor influencing vitamin D status. Diet, body weight, age, clothing, time outdoors, latitude, supplementation, and individual biology can all be important.

Photoprotection guidance has also become more nuanced. Darker pigmentation provides considerable natural ultraviolet protection but does not eliminate ultraviolet damage. Sunscreen, protective clothing, shade, and other measures may still be useful, particularly for people with photosensitive disorders, pigmentary conditions, a history of skin cancer, or prolonged ultraviolet exposure.

Visible light can also contribute to pigmentation disorders in darker skin, making broad-spectrum approaches to photoprotection relevant to conditions such as melasma and post-inflammatory hyperpigmentation.

Skin Classification and Measurement

Research increasingly questions the widespread use of race or the Fitzpatrick skin-type system as simple substitutes for measured skin color.

The Fitzpatrick classification was originally designed primarily to characterize sunburn and tanning responses. It was not intended to serve as a universal racial classification or a precise measurement of constitutive pigmentation.

Objective approaches such as reflectance measurements, colorimetry, spectrophotometry, and standardized skin-tone scales may provide more useful information when pigmentation itself is the variable being investigated.

This distinction is especially important for studies of pulse oximetry, wearables, imaging technologies, ultraviolet responses, and artificial intelligence. Using broad racial categories can conceal considerable variation in pigmentation within populations and can incorrectly imply that race is a biological measurement.

Healthcare Access and Research Equity

Differences in health outcomes cannot be explained by pigmentation alone. Access to healthcare, insurance, income, geography, language, transportation, education, discrimination, and availability of specialists can strongly influence diagnosis and treatment.

Research has found racial and ethnic disparities in access to dermatologists, barriers to treatment for chronic inflammatory skin diseases, management of skin ulcers, and participation in clinical research.

People with darker skin have also historically been underrepresented in dermatology clinical trials. Inadequate reporting of race, ethnicity, and skin tone can make it difficult to determine whether therapies and diagnostic tools have been adequately evaluated across diverse populations.

Specialty clinics focused on skin of color may help address some gaps by providing clinicians with greater experience recognizing conditions across different pigmentation levels. However, broad improvement requires incorporating diverse populations into routine healthcare, education, research, product development, and regulatory standards.

Teledermatology and Digital Health

Teledermatology has the potential to expand specialist access, but it can also reproduce existing inequalities.

High-quality dermatologic assessment by photograph or video depends on adequate cameras, lighting, internet access, digital literacy, and image quality. Studies have identified differences in teledermatology participation and photograph submission associated with race, age, insurance status, and other social factors.

Image quality can also interact with pigmentation. Cameras, automatic exposure systems, compression algorithms, and lighting conditions may not reproduce all skin tones equally well.

Digital-health systems therefore need to be designed not merely for technical convenience but for equitable usability across different populations and skin colors.

Colorism and Population Health

Colorism refers to social advantages or disadvantages associated with lighter or darker skin tone, often operating within populations that outsiders might classify as a single racial or ethnic group.

Studies in the United States, Latin America, and other regions suggest that skin tone can influence experiences of discrimination, socioeconomic status, social treatment, and health. Research has linked colorism with self-rated health, hypertension, psychological distress, depression, sleep, and stress-related biological pathways.

These findings illustrate why public-health research sometimes needs more precise measures than broad racial categories. Two people identified as members of the same racial group may have substantially different experiences because of differences in perceived skin tone.

At the same time, researchers must distinguish between biological effects of pigmentation and health effects created by discrimination. Skin color itself does not biologically cause the social disadvantages associated with colorism; rather, social systems assign different meanings and treatment to visible differences in appearance.

Skin Lightening and Global Public Health

Skin-lightening or skin-bleaching practices are widespread in parts of Africa, Asia, the Middle East, the Americas, and other regions. Their popularity reflects a complex interaction of beauty standards, colonial history, advertising, social status, gender expectations, employment opportunities, marriage preferences, and colorism.

Some skin-lightening products contain medically hazardous ingredients. Research has identified products containing excessive concentrations of mercury, hydroquinone, and potent corticosteroids. Use can lead to irritation, abnormal pigmentation, skin thinning, infections, steroid-related complications, mercury toxicity, and other systemic health effects.

Product labeling is not always reliable, and unregulated or counterfeit products can circulate internationally through informal markets and online commerce.

Because demand is driven partly by social pressures surrounding skin color, regulation alone is unlikely to eliminate harmful use. Effective public-health responses may require product regulation, consumer education, clinical counseling, enforcement against hazardous ingredients, and broader efforts to challenge discriminatory beauty standards.

Toward More Equitable Healthcare

Improving health equity across skin tones requires changes throughout healthcare and biomedical research.

Clinical education should routinely demonstrate how diseases appear across diverse pigmentation. Medical devices should be tested using objectively measured skin tone and across clinically relevant conditions. Artificial-intelligence systems should be trained and evaluated on representative datasets. Clinical trials should include diverse populations and report demographic and pigmentation information clearly.

Public-health programs should also distinguish biological pigmentation from racial identity. Race can remain important when studying discrimination, structural inequality, healthcare access, and social determinants of health, but it should not automatically be treated as a biological proxy for skin color.

Greater precision in terminology and measurement can help researchers identify whether a disparity arises from physiology, technology, healthcare systems, socioeconomic conditions, discrimination, or some combination of these factors.

Conclusion

Skin color intersects with public health through both biology and society. Pigmentation influences ultraviolet responses, vitamin D production, disease appearance, optical measurements, and some aspects of dermatologic risk. At the same time, discrimination, colorism, unequal healthcare access, inadequate representation in medical education, research disparities, and social pressures surrounding skin tone can produce important differences in health and well-being.

The growing literature on skin color demonstrates the limitations of using broad racial categories as substitutes for pigmentation. More equitable healthcare requires objective measurement when skin pigmentation is biologically relevant, greater representation across skin tones in medical education and research, improved testing of medical devices and artificial intelligence, and stronger attention to the social conditions that convert visible differences in skin color into differences in health.
