Skin Diseases Across Populations
Human skin diseases occur in every population, but their frequency, appearance, severity, complications and treatment outcomes are not necessarily identical. Dermatological research increasingly shows that skin pigmentation, genetics, hair structure, climate, environmental exposure, cultural practices, socioeconomic conditions and access to health care can all influence patterns of skin disease.
Differences between populations should not be interpreted as simple biological divisions based on race. Race and ethnicity are imperfect proxies for a complex combination of ancestry, geography, pigmentation, environment, culture and social conditions. Individuals within the same racial or ethnic category can have widely different skin tones and disease risks.
One of the most important themes in modern dermatology is therefore the need to distinguish skin pigmentation from assumptions about ancestry while recognizing that some diseases, complications and health-care disparities disproportionately affect particular communities.
Skin Tone and the Appearance of Disease
Many dermatological diagnoses have historically been taught using photographs of light skin. This creates difficulties because inflammation, redness and other classic diagnostic signs may look substantially different in deeply pigmented skin.
Erythema, which is usually described as redness, may instead appear violet, gray, brown or only subtly darker than surrounding skin. Inflammation may consequently be underestimated if clinicians rely primarily on visible redness.
Pigmentation can also influence what remains after inflammation resolves. Post-inflammatory hyperpigmentation may leave dark patches that persist much longer than the original acne, eczema, injury or other inflammatory condition. Hypopigmentation may similarly develop following inflammation or disease.
These differences have encouraged greater use of dermoscopy, improved photography, objective skin-color measurements and other diagnostic techniques that depend less on subjective visual descriptions.
Traditional systems such as the Fitzpatrick skin-type scale have also been criticized because they may combine skin pigmentation with assumptions about sun sensitivity. Modern approaches increasingly emphasize measuring pigmentation and ultraviolet response independently.
Atopic Dermatitis, Psoriasis and Other Inflammatory Diseases
Atopic dermatitis, or eczema, is one of the most extensively studied examples of how dermatological disease can vary among populations.
Eczema in darker skin may display papular, lichenoid, violaceous or strongly pigmentary changes rather than the bright red appearance frequently illustrated in textbooks. These variations can complicate diagnosis and assessment of disease severity.
Research has also reported differences among populations in eczema prevalence, skin-barrier characteristics, immune responses and health-care utilization. Environmental exposures, socioeconomic conditions and access to specialty care can contribute alongside biological factors.
Psoriasis likewise affects people of every skin tone. In darker skin, plaques may appear violaceous, gray or dark brown rather than distinctly red. Pigmentary changes can remain after active plaques disappear.
Studies of psoriasis and atopic dermatitis have also highlighted inadequate representation of some racial and ethnic populations in clinical trials. Poor representation can make it difficult to determine whether treatment evidence applies equally across populations.
Hidradenitis suppurativa, prurigo nodularis, seborrheic dermatitis and chronic pruritus are additional inflammatory disorders for which differences in prevalence, clinical appearance or disease burden have been reported.
Acne and Pigmentary Disorders
Acne is extremely common across human populations, but its consequences may differ according to pigmentation.
For many people with darker skin, post-inflammatory hyperpigmentation can be as troublesome as the active acne itself. Treatment therefore often requires controlling inflammation while minimizing irritation that could cause additional pigmentation.
Retinoids, hydroquinone, chemical peels, lasers and other treatments can improve hyperpigmentation, but aggressive procedures may occasionally worsen discoloration. Treatment strategies consequently need to account for both the underlying disease and the patient's response to inflammation.
Other pigmentary disorders receiving increasing attention include melasma, lichen planus pigmentosus, progressive macular hypomelanosis and disorders involving partial or complete pigment loss.
Vitiligo occurs across populations but can be particularly visually prominent when affected areas contrast strongly with surrounding deeply pigmented skin. Research from different societies demonstrates that the condition can have substantial psychological and social consequences, including stigma and social exclusion.
The burden of a pigmentation disorder therefore cannot always be measured simply by the physical area of skin involved.
Hair and Scalp Disorders
Hair structure and hair-care practices can influence dermatological conditions, making culturally informed care especially important.
Central centrifugal cicatricial alopecia, or CCCA, is a scarring hair-loss disorder reported predominantly among women of African descent. Research has examined possible contributions from genetics, inflammatory pathways, hairstyling practices, metabolic conditions and environmental influences.
Because CCCA destroys hair follicles, delayed diagnosis can result in permanent hair loss.
Traction alopecia can develop when repeated tension is applied to the hair. Tight braids, extensions, weaves and other styles can contribute when they place prolonged stress on follicles. Early recognition and changes in hairstyling may help prevent permanent damage.
Pseudofolliculitis barbae is another condition closely connected with hair structure. Curved hairs can re-enter the skin after shaving, producing inflammation, ingrown hairs, pigmentation and sometimes scarring. It is particularly common among people whose facial hair is tightly curled.
Dermatological care of hair and scalp disease therefore benefits from knowledge of hair texture, grooming practices and cultural traditions rather than treating hair simply as an isolated biological feature.
Autoimmune and Systemic Diseases
Several autoimmune diseases produce skin manifestations that may appear differently across skin tones or occur at different frequencies among populations.
Cutaneous lupus can cause inflammation, pigmentary changes, scarring and permanent hair loss. Studies have reported substantial burdens of discoid lupus and associated skin damage among Black patients.
Dermatomyositis can likewise be more difficult to identify when educational materials emphasize characteristic redness on light skin. Studies of medical images have found inadequate representation of darker skin in materials used to teach recognition of dermatomyositis.
Autoimmune blistering diseases such as pemphigus and pemphigoid can also differ substantially in incidence among populations. Research comparing ethnic groups living in the same geographic regions suggests that genetic susceptibility may contribute to some of these differences.
Sarcoidosis is another systemic disease with important dermatological manifestations. Cutaneous forms vary widely in appearance, and the disease has been reported more frequently among some populations, including people of African ancestry.
Infectious and Tropical Skin Diseases
Patterns of infectious skin disease are strongly affected by geography, climate, living conditions and access to health care.
Fungal infections such as tinea capitis show substantial population differences in some studies. These patterns may reflect exposure, household transmission, hair-care practices, socioeconomic conditions and other environmental factors rather than ancestry alone.
Leishmaniasis, leprosy, scabies and many bacterial, viral and fungal infections are particularly important in global dermatology. Their distribution often corresponds more closely to ecology, poverty, migration and health-system capacity than to skin color.
Indigenous communities can face elevated burdens of infectious and inflammatory skin disease when populations experience inadequate water supplies, sanitation problems, overcrowding, environmental disruption or limited access to medical services.
Refugees and migrants may similarly experience skin disease related to overcrowding, interrupted health care, occupational exposure, climate changes and conditions encountered during displacement. Dermatological health in migrant populations therefore illustrates the close relationship between skin disease and broader social conditions.
Skin Cancer and Population Differences
Skin cancer occurs in people of every skin tone, although incidence, typical anatomical locations and relative risks differ substantially.
Melanin provides some protection against ultraviolet radiation, and many common ultraviolet-associated cancers occur less frequently in people with deeply pigmented skin. Lower incidence, however, does not mean absence of risk.
Melanoma in people with darker skin is sometimes diagnosed at a later stage. Acral melanoma, which occurs on palms, soles and around nails, represents an important diagnostic concern because it can develop in areas receiving comparatively little ultraviolet exposure.
Research has also identified population differences in outcomes for melanoma and other skin malignancies. Delayed recognition, differences in access to dermatologists, disease biology and socioeconomic factors may all contribute.
Dermatofibrosarcoma protuberans, a rare skin cancer, has been reported at higher incidence among Black populations in United States cancer registries.
Cutaneous T-cell lymphoma, including mycosis fungoides, is another malignancy in which studies have identified differences in presentation and outcomes among racial and ethnic populations.
Albinism and Skin Cancer
Albinism provides one of the clearest examples of the relationship between pigmentation and ultraviolet susceptibility.
People with oculocutaneous albinism have greatly reduced melanin protection. In areas with intense ultraviolet radiation, particularly parts of sub-Saharan Africa, this creates a substantial lifetime risk of actinic damage and skin cancer.
Studies from African countries have documented high rates of squamous cell carcinoma and other ultraviolet-associated malignancies among people with albinism. Cancers may occur at relatively young ages and can become advanced when access to prevention, screening and treatment is limited.
Photoprotective clothing, hats, sunscreen, education, regular skin examinations and timely treatment of precancerous lesions can substantially reduce this burden.
The experience of people with albinism demonstrates how biology, geography, poverty and health-care access can interact to determine disease risk.
Children and Skin Disease
Children with different skin tones can develop the same broad range of dermatological diseases, including eczema, acne, impetigo, scabies, fungal infections and pigmentary disorders.
However, childhood skin disease may have different appearances in darker skin. Eczema, inflammatory disorders and infections can be overlooked when clinicians have been trained primarily with images of light-skinned patients.
Hair disorders, traction alopecia, tinea capitis and pigmentary conditions are also important components of pediatric dermatology in diverse populations.
Research has identified disparities in diagnosis, referrals, specialist access and medical-resource representation. Improving pediatric dermatology therefore requires both better clinical training and attention to socioeconomic and structural barriers.
Dermatology Education and Representation
A recurring concern in dermatology is the historical underrepresentation of dark skin in medical textbooks, journals, examination materials and educational resources.
Multiple studies have found that clinical images disproportionately depict lighter skin. This can reduce clinician confidence and make recognition of disease in darker skin more difficult.
Efforts to diversify medical curricula have shown that exposure to broader collections of clinical images can improve students' confidence in diagnosing dermatological disease across skin tones.
Representation is also important in clinical research. Trials that fail to report participant skin color, race or ethnicity make it difficult to determine whether study findings apply equally across populations.
The goal is not to create entirely separate systems of dermatology for different racial groups. Rather, medical education should demonstrate how the same diseases may appear across the full range of human skin pigmentation.
Artificial Intelligence and Diagnostic Bias
Artificial intelligence is increasingly being developed to identify skin diseases from photographs.
These systems offer potential benefits, including improved diagnostic support and wider access to dermatological expertise. However, their reliability depends heavily on the images and patient populations used for training.
Studies have demonstrated that some dermatology AI systems perform less accurately on darker skin or uncommon diseases. If training data disproportionately represent light skin, automated diagnostic tools can reproduce or amplify existing inequalities.
Researchers are developing more diverse image datasets and methods designed to reduce skin-tone bias. AI therefore has the potential either to narrow dermatological disparities or reinforce them, depending on how systems are developed and evaluated.
Health Disparities and Access to Care
Differences in dermatological outcomes cannot be explained by pigmentation or genetics alone.
Insurance coverage, income, geographic location, specialist availability, medical education, language, cultural practices, occupational exposure and trust in the health-care system can all influence whether a skin condition is recognized and treated.
Black, Indigenous, migrant and other historically underserved populations may experience delayed diagnosis or reduced access to dermatologists. Geographic isolation can be especially important for Indigenous and rural communities.
Environmental conditions also influence dermatological health. Climate change, heat, air pollution, ultraviolet exposure, flooding and wildfire smoke may worsen certain skin diseases, while communities with fewer resources can face greater difficulty adapting to these hazards.
Understanding skin disease across populations therefore requires combining dermatology with public health, environmental science and the social determinants of health.
Moving Beyond Race-Based Dermatology
Population differences remain important for clinical medicine, but broad racial categories can conceal enormous diversity.
Two people classified within the same racial group may differ greatly in pigmentation, ancestry, environmental exposure and health risks. Conversely, people from different ethnic backgrounds may have similar skin pigmentation and similar dermatological responses.
More precise research increasingly attempts to separate measurable characteristics such as pigmentation, hair morphology, genetic variants, ultraviolet response and environmental exposure from broad racial labels.
This approach can preserve clinically useful information about population differences without assuming that socially constructed racial categories represent fixed biological divisions.
Conclusion
Skin disease reflects the extraordinary diversity of human biology and human environments. Pigmentation can alter the appearance and consequences of inflammation, hair structure can influence scalp disease, genetic variation can affect susceptibility to particular disorders, and ultraviolet exposure can dramatically alter cancer risk.
At the same time, many of the largest differences in dermatological health arise from social circumstances rather than biology alone. Access to specialists, representation in medical education, environmental conditions, migration, poverty and the availability of preventive care can substantially alter disease outcomes.
Modern dermatology is therefore moving toward a more inclusive model that recognizes variation across skin tones and populations while avoiding simplistic assumptions about race.
Improved clinical imagery, representative research populations, culturally informed treatment, objective measures of skin characteristics and carefully validated artificial-intelligence systems can make diagnosis more accurate for everyone.
Understanding skin diseases across populations ultimately demonstrates a broader principle of medicine: human biological variation matters, but it must be interpreted together with environment, culture and social conditions rather than reduced to racial categories alone.
